Higher income can affect more than your tax bill. It can also increase your Medicare premiums.

IRMAA stands forIncome-Related Monthly Adjustment Amount. It is an additional monthly charge applied to Medicare Part B and Part D for beneficiaries whose income exceeds certain thresholds.

For 2026, IRMAA generally applies when 2024 modified adjusted gross income exceeds:
• $109,000 for an individual filer
• $218,000 for a married couple filing jointly

Why does this matter for retirement planning?

Certain financial decisions, including Roth conversions, retirement-account withdrawals, and realizing capital gains, may increase income and potentially affect Medicare premiums two years later.

IRMAA also works like a series of income brackets. Crossing a threshold can move a beneficiary into a higher monthly premium level. That makes it important to evaluate taxes, investment income, and Medicare costs together.

The goal is not necessarily to avoid IRMAA at all costs. It is to understand the potential cost before making a decision and determine whether the overall strategy still makes sense.
